Glucocorticoid for Nephrotic Syndrome: It May Make You DisabledGlucocorticoid is a commonly used treatment for Nephrotic Syndrome patients. It can help reduce proteinuria and control the progression of disease, but it has may side effects, such as moon face, buffalo back, stretch marks, obesity (increased appetite), acne, increased blood pressure, elevation of blood sugar, etc. Most of the side effects of the hormone will be recovered after the drug is discontinued, and there is no serious harm. However, there is one side effect to be guarded: femoral head necrosis. The incidence is not high, but extremely dangerous. In serious case, it can make people disabled.

About glucocorticoid

1. Short acting hormones with the name "cortisone," such as cortisone and hydrocortisone (produced by the body itself).

2. Medium-acting hormones with the name "predni", such as prednisone, prednisolone and methylprednisolone.

3. Long-acting hormones with the name “metha”, such as dexamethasone and betamethasone.

Over 20mg of glucocorticoid per day, femoral head necrosis may occur, with the highest incidence between 5 and 8 months. Before using hormones, the indications should be strictly controlled.

How to detect steroid associated femoral head necrosis?

Steroid-induced necrosis of the femoral head can be completely asymptomatic at an early stage. If you have typical symptoms, such as hip pain, or stiffness, or a slight inconvenience in walking, you should have a further examination immediately to make sure.

You can do a hip X-ray or CT, but MRI is more accurate and reliable in early diagnosis. If the diagnosis has been made, subsequent review and follow-up will not require mri, X-ray or CT is OK.

Prevention and treatment of femoral head necrosis

1. Salmon Calcitonin Acetate + Osteoform

This combination can prevent and promote the repair of early steroid-induced femoral head necrosis. There are few cases in this area, which need further study.

2. Chinese medicine: promoting blood circulation to remove blood stasis + reducing phlegm

In the use of hormones, the use of removing phlegm and blood stasis with the treatment of traditional Chinese medicine, such as ling gui shu gan tang, and siwu tang, can prevent the occurrence of steroid-induced femoral head necrosis. A clinical study has shown that traditional Chinese medicine can reduce the risk of femoral head necrosis to 20% without any adverse effects on the liver and kidney function.

However, the results of these treatments are not very satisfactory. For patients with severe femoral head necrosis, artificial hip replacement can be performed.

In summary, nephrotic syndrome patients must master the indications and weigh the advantages and disadvantages before using hormones. And examination should be done as early as possible to detect signs of femoral head necrosis, reducing the risk of disability.
