FSGS with Collapsing Type: What Else I Can Do Besides Anti-hypertension

Question: I was diagnosed with Focal Segmental Glomerulosclerosis (FSGS) with collapsing feature. I am 47 years old female with history of high blood pressure, and I am on several b/p medications. Please advice. Is there anything else I can do?

FSGS with Collapsing Type: What Else I Can Do Besides Anti-hypertensionAnswer: FSGS with Collapse type, I must to tell u, is the most serious one compared with the other types. I think u should have the Diarrhea or upper respiratory tract infection before get it. It onsets quickly and the development is also fast, as usual, after 1 or 2 years it will lead to the kidney failure.

The main cause of FSGS is that glomerular epithelial cells have been damaged. It is the auto-immune disease, when our immunity is low, it is easy to lead the immune disorder, the immune disorder lead to the Allergic immune response, then too many complexes such as the IGM, C3 deposited in the kidney, lead to the poor blood and oxygen of the kidney then damage the epithelial cells. When the glomerular epithelia has been damaged, it will affect the barrier function then leading to the leakage of large amounts of protein and red blood cells. What is more,the damaged epithelia will bring the inflammatory factor then it will aggravate the damaged kidney. So if we want to treat FSGS thoroughly, we must block the allergic immune response and repair the damaged renal epithelial cells.

Now you just take the medicine to control your hypertension. In terms of the damaged epithelia, you do not have the treatment for it.

The general western treatment for FSGS is to use the hormone firstly. If the patients are not sensitive to it, they must take CsA, but the CsA has the renal toxicity, even if the Tacrolimus, it just can reduce the protein in urine because it can block the immune reaction, but it can not remove the deposited IGM, C3, so the damaged epithelia cells can not been repaired.
