Recurrence of FSGS After Kidney Transplant

Question: My sister has FSGS. She has done kidney transplant. But recently she started to have proteinuria. She still has normal kidney function, but we are afraid it could be a recurrence of the FSGS. Please give us some suggestions.

Recurrence of FSGS After Kidney TransplantAnswer: Proteinuria is a typical symptom of kidney disease. Since your sister has such a symptom, it means her kidneys fail to work again. But her normal kidney function tells us that she is in early stage.

There are many reasons why a kidney transplant can fail.

-Chronic rejection. This is the most common reason that kidney transplants fail. It is the long term damage done by the body’s immune system for a lot of different reasons. How many years of kidney transplant has your sister done?

-Recurrent disease. For other types of kidney disease, it is not very common, but for FSGS patients, recurrence of FSGS is seen in about 30%-40% of patients with primary FSGS who undergo transplantation. Recurrent FSGS occurs within the 1st year of transplant in about 80% of patients and may even manifest within hours after transplantation.
